Work Related Injury
  1. When a work-related injury or accident occurs, the appropriate procedures are as follows:
    1. Report the incident to the appropriate supervisor, the Security Office, and the Human Resources Office.
    2. Immediately check with one of contacts listed above in order to secure the name of an approved medical provider. NOTE: Security will fill out an incident report. In addition, an accident report form (available from Security or Human Resources) must be completed by the employee and the supervisor. Upon completion, this form is either mailed or faxed to Sedgwick Claims Management Services, Inc., c/o Ms. Kathy Dawson, P. O. Box 19808, Knoxville, TN 37939-2808. Their fax number is 1-865-588-1153. A copy of the accident report form also needs to be sent to the Human Resources Office.
  1. With work-related injuries and accidents, you must choose a medical provider from the state of Tennessee approved provider directory in order to ensure full payment of the bills that you might incur. In order to secure the name of an approved medical provider, you may call the state of Tennessee's administrator of workers' compensation, Sedgwick Claims Management Services, Inc. at 1-800-526-2305 or USA Workers' Injury Network at 1-800-872-3860. A printed copy of the provider directory is also available on the state of Tennessee Treasury website as follows: A wallet-sized card with the contact numbers is available in Human Resources upon request. In cases of work-related incidents, it is imperative to access these contacts each time treatment is sought since the list of providers is subject to change. If you use an unauthorized provider, you will be responsible for payment of your bills.
